Northern Light Health is seeking a community health worker to help patients overcome barriers, access resources, and take meaningful steps toward better health and well-being.

In this role, you’ll build trusted relationships with patients of Northern Light Maine Coast Hospital in Ellsworth in their homes, healthcare settings, and communities while helping them:

·         Connect with healthcare and community resources

·         Set and work toward health goals

·         Access benefits, food assistance, housing resources, prescriptions, and other support

·         Navigate appointments and the healthcare system

·         Address social challenges that may impact their health

We’re looking for an empathetic listener, strong communicator, and natural relationship-builder who is comfortable with outreach and home visits.
